In short

• We only ask for the details we need to contact you, review a referral or consider a job application.
• Our forms never ask for diagnoses, medications or medical history, and we ask you not to include them.
• We don’t sell your information or use it for advertising.
• Only authorized Central Healthcare Services staff can see what you send us.

What we collect

When you request care or send us a message

Your name, phone number, email address (if you give it), who the care is for, the county where care is needed, the service you’re interested in, how and when you’d like us to contact you, and any short message you choose to write.

When you send a referral

Your name, role, organization and contact details, plus the name, phone number (if given) and county of the person you are referring, the service needed, a good time to reach them and any short scheduling notes. Please only send a referral if you have that person’s permission, or are otherwise authorized, to share their contact details with us.

When you apply for a job

Your name, phone number, email address, the county you live in, the role you’re interested in, your availability, any message about your experience and, if you choose to upload one, your resume.

Technical information

To protect our forms from spam, we record a one-way scrambled (hashed) version of your internet address for a short time so we can limit repeated submissions. We cannot turn it back into your address and we delete these records within a day. Our hosting providers also keep standard server logs for security and reliability.

How we use it

• To contact you about the care, referral, message or job you asked about.
• To arrange and coordinate services if you decide to work with us.
• To keep a record of inquiries so our team can follow up properly.
• To keep the website secure and prevent spam and abuse.

We do not sell or rent personal information, and we do not use it for advertising. If you become a client, your health information is handled separately under the privacy practices we give you at the start of care.

Who can see it

Submissions are stored in a secure database that only authorized Central Healthcare Services staff can access, using individual password-protected accounts. Resumes are stored privately and can only be opened by staff through short-lived links.

We use a small number of service providers to run this website:

• Vercel, which hosts the website.
• Supabase, which provides the secure database and file storage for form submissions.
• Resend, which sends our staff a short email alert (name, phone number and type of submission) when a form is sent.
• Google, which provides the map on our contact page.

These providers process information on our behalf to provide their services. We may also share information if the law requires it.

Cookies

This website doesn’t use advertising or tracking cookies. Staff who sign in to our private inbox use a secure sign-in cookie. The embedded Google map on our contact page is provided by Google and may set its own cookies under Google’s privacy policy (opens in a new tab).

How long we keep it

We keep inquiries and referrals for as long as we need them to respond and to keep accurate records, and job applications for up to two years so we can contact applicants about future openings. You can ask us to delete your information at any time, unless we are required to keep it.

Your choices

You can ask to see, correct or delete the information you sent through this website. Contact us using the details below and we’ll respond as soon as we can.

Children

This website is meant for adults. If you are arranging care for a child, a parent or guardian should contact us.

Emergencies

Our forms are not monitored around the clock. If this is a medical emergency, call 911.
