Skilled nursing

Some needs call for a licensed nurse. Skilled nursing brings that clinical knowledge into the home, so care can happen in familiar surroundings.

How skilled nursing helps

Our nurses provide health-related care at home as directed by a plan of care and your loved one’s physicians. They watch for changes, keep the care team informed, and help families understand what to look out for between visits.

Skilled nursing often works alongside personal care. A nurse can oversee the clinical side while a caregiver supports daily routines, giving families one team to talk to instead of several.

We are glad to speak with physicians, discharge planners and case managers to coordinate care. When you contact us, we will talk through what has been recommended and how our nurses can help.

What skilled nursing can include

  • Nursing care from registered or licensed practical nurses
  • Medication management and administration
  • Wound care and dressing changes
  • Monitoring after surgery or a hospital stay
  • Support managing long-term health conditions
  • Coordination with doctors, case managers and other providers

Every plan is different. We’ll talk through exactly what support is right before care begins.

Skilled nursing may be a good fit if

  • A physician has recommended nursing care at home
  • Someone is returning home from a hospital or care facility
  • A family wants clinical oversight alongside daily support
