Rehabilitation support

Recovery doesn’t stop when someone leaves the hospital. Rehabilitation support helps people rebuild strength, confidence and independence in the place they know best.

A caregiver holds an older woman’s hands as she rises from her armchair, with her walker beside her.

How rehabilitation support helps

After an illness, a fall, surgery or a hospital stay, the weeks at home make a real difference. Our team helps with safe movement around the house and supports the routines and exercises recommended by your loved one’s therapists and doctors.

We focus on confidence as much as mobility. Small wins, like walking to the mailbox again or getting ready without help, are worth noticing, and a steady presence can make them feel possible.

Rehabilitation support works well alongside personal care and skilled nursing. We will talk with you about what the care team has recommended and how we can fit in.

What rehabilitation support can include

  • Help with walking, transfers and moving safely at home
  • Encouragement with exercises recommended by therapists
  • Fall prevention and home safety guidance
  • Setting up routines that build strength and independence
  • Support getting to and from appointments, where arranged
  • Keeping the family and care team informed of progress

Every plan is different. We’ll talk through exactly what support is right before care begins.

Rehabilitation support may be a good fit if

  • Someone is home after a hospital stay, surgery or fall
  • Mobility or balance has changed after an illness
  • A therapist has recommended practice and routines at home
